Job Description

Are you an experienced RN with a strong understanding of MDS and care planning? Napa Post Acute is seeking a detail-oriented RN MDS Coordinator to lead our assessment and care planning processes with accuracy, compassion, and clinical excellence. What You’ll Do: Coordinate and oversee the MDS assessment process to ensure timely and accurate submissions; Develop and update individualized care plans with the interdisciplinary team; Ensure compliance with all federal and state regulations, including PDPM guidelines; Collaborate with clinical and administrative staff to optimize resident outcomes and reimbursement; Provide staff education related to MDS processes and documentation. What We’re Looking For: Active Registered Nurse (RN) license in California; Minimum 1 year of MDS experience in a skilled nursing facility; Strong knowledge of PDPM, CMS regulations, and RAI process; Excellent organizational, communication, and critical thinking skills; Passion for delivering quality care to seniors.
